SELF-PAY
We are a self-pay practice. This means we serve our patients directly instead of serving 3rd party insurance companies.
When you use insurance at most medical practices, you don't know what the office visit or procedure will cost until a large bill shows up in your mailbox.
Our pricing is more transparent, upfront, and reasonable. You will know what your visit will cost ahead of time.
If a procedure needs to be performed during your visit, we will let you know before performing the procedure what the additional charge would be.
You can still use insurance to pay for prescriptions and lab tests. However, we also dispense low-cost medications directly from our office and can order blood tests for you at very low cost.
Pricing
Please call our office for office visit pricing depending on your needs.
Payment
Payment is due at checkout. We accept the following:
Cash
Check
Visa, Mastercard, or Discover
HSA (health savings account)
FSA (flexible spending account)
Can I Submit my Bill to my Insurance Company?
Yes. We will provide you with an itemized and coded receipt that you can submit to your insurance company for reimbursement.
Since we are not contracted with any insurers, we are "out-of-network" on all insurance plans. It is your responsibility to check with your insurance provider to determine if you will be reimbursed for out-of-network expenses.
Government insurances such as Medicare and Medicaid do not allow a claim to be submitted.
Request an Appointment
Please book your appointment online or call our office (847) 615-8700.
If you are brand new to our office we require a $100 deposit to hold your first appointment. The deposit will be applied to the cost of the appointment on the day of your visit. If a patient does not show for their first appointment or if the patient cancels without giving one business day’s notice, the deposit will be forfeited. Missed appointments cause our office time-allocation problems and prevents other patients from getting timely appointments.
Cosmetic patients: We require a non-refundable $200 deposit for a filler appointment since this is a longer appointment. The deposit will get deducted from your total fee on the day of your filler treatment. Should you decide not to proceed with the filler treatment after arriving for your appointment, the $200 deposit will be forfeited and applied to the 30 minute block of time taken up on the doctor’s schedule. Last-minute cancellations cause time-allocation problems and prevents other patients from getting a timely appointment.
Cancellation Policy
Missed appointments cause scheduling and time-allocation problems and also denies another patient timely access to our office. If you cannot keep an appointment, we kindly request you cancel with at least one business day's notice.
For appointments that are 45 minutes or longer, we kindly request that you cancel with at least three business days’ notice.
Except for cases of emergency, you will be charged the full price of your appointment if you do not show or if you cancel at the last minute.
